0 Quick Apply Full-time 1 hour ago Full Job Description Job description The basic function of this position is to work with the clinical teams to provide patients with continuous, coordinated and comprehensive health services.
Essential Duties and Responsibilities Demonstrates and applies knowledge of the principles of comprehensive, patient centered, culturally sensitive care coordination of services Participates in organizational Quality Improvement program, utilizing plan, do study, act (PDSA) methodology to test change and improve outcomes.
Ability to assist patients with barriers to help navigate health care systems Develop and maintain effective working relationships with Promise Healthcare's providers.
Maintains high data integrity Comfortable working in an environment that has high performance expectations Excellent attention to detail and follow through on assignments Ability to adapt to changing work environment and duties Works collaboratively with the referral coordinators to tracks patient progress throughout the referral and tasking process.
In conjunction with the referral coordinators provides follow up on incomplete referrals and develops processes to improve by monitoring referrals and tracking patient follow through Maintain recall system for patient follow-up, using patient population health module Monitor and process required program documentation Document progress and activity notes corresponding to all patient contacts Schedule patient appointments as needed.
Manages UDS metrics and provides information to provider teams.
Submit documents and reports in a timely manner - at least monthly to Supervisor Education and/or Requirements:
Experience at an FQHC is highly preferred Previous experience working with UDS and Quality metrics Bachelor's degree or higher required or Specific Vocational Preparation (2 years of equivalent specific experience exchanged for bachelor's degree) High School Graduation or Equivalent required.
